Camp Freedom

NB: Please submit one form per child. Please ensure all questions marked with an asterisk (*) are completed in full.

We understand that staying away from home can be difficult for people of all ages. Campers who would prefer to stay overnight at home and attend camp during the day only may do so. (Drop off and pick up options will be communicated prior to camp)

This information is required if you are unable to collect your child for any reason.

Parents or guardians MUST be available to collect their child from camp at any time during the camp period, including outside normal hours.

This form is of vital importance to the safety and wellbeing of your child. In an emergency, it may be the only information that staff/leaders or health professionals have for the care and/or treatment of your child. Please take time to personally fill it in and include any and all information you believe may be relevant. Failure to disclose information may impact on your ability to attend AOWA camps in the future. Please remember that AOWA is not a medical service provider and may be unable to cater for campers with medical or other conditions that require certain support.

AOWA would like to provide your child with a sense of feeling safe at camp.

Please note: All medications will be held by the camp nurses, where medication will be provided at the appropriate time.
